- First, we would like to welcome you to come and take a tour of our community! Once here you can meet with Jamie Varner, our Admission Coordinator or one of our Quality of Life Team Members. To set up a scheduled visit please contact Jamie by phone at (913)477-8235.
- At your visit with us you will asked to sign a Release of Information. This allows our community to request documentation from current caregivers/physicians and have it sent to Evergreen Community via fax. Our fax number is (913)477-8001.
- Once a room is available Jamie and a nurse will come and visit with your family member. If your family member lives outside of the Kansas City Metro area Jamie and the nurse will ask to speak to current care givers, physicians, etc... in order to collect further information.
- Upon coming to live in our community your family member will need to apply for a Kansas CARE Assessment. It can be request by calling the Area Agency on Aging at (913)715-8857. The CARE Assessment is a requirement of the federal government that assures the potential resident is appropriate for long term care placement in a facility such as Evergreen. This assessment must be done prior to admission and can take up to 10 days to obtain, so please call ASAP.
- Caregivers fax us the requested information.
- Our community receives the CARE assessment summary. The Facility Admissions Committee meets and reviews all of the information obtained from the visit with your family member and the records from the caregivers/physicians. A decision is then made if our community can meet your family member's needs.
- A staff member will notify you of the decision. If the admission has been approved, a date and time will be set.
- A staff member will give you or your family member a list of the medical information that will need to be brought at the time of admission.
- Bring in any family photos, a favorite chair, and such. Remember to Make it HOME!

Jamie comes to our community with many years of experience, in which she served as a Social Service Consultant for the past 3 years. Prior to that she was Social Service Director.
